以全科门诊为主要场所的全科专业住院医师培养模式探索

摘　　要：香港大学深圳医院(简称我院)是公立医院改革的“试验田”,以深港合作为契机,将中国香港大学医学院、中国香港医学专科学院的经验带到内地,为医学人才培养带来全新理念、提供有益借鉴。该院全科医学科秉承“先全科后专科”的服务理念,年门诊量超过10万,为医学人才培养提供了丰富的临床、教学、科研资源。住院医师从观察、监督下参与、独立完成诊疗的阶梯式工作模式,从培养“看病”临床思维基本能力,到“看好病”疾病规范化诊疗,最后到“看好病人”全人照顾及综合能力培养。主要措施有入门引领,早全科,早基层;每周三定期回全科门诊工作及学习,增加家庭归属感;开展以学员为中心的教学策略;成立多学科教研室,推动全科住院医师在其他专科轮转;落实导师制;派优秀学员到中国香港公立、私立医院和门诊进一步学习,增加深港地区间交流,拓宽全科视野等。The University of Hong Kong-Shenzhen Hospital serves as an"experimental field"of public hospital reform.With the Shenzhen-Hong Kong collaboration,it is able to bring the experience of University of Hong Kong's Faculty of Medicine and the Hong Kong Academy of Medicine to Mainland of China.Innovative ideas and useful reference materials are incorporated in the training of medical talents.The General Medicine Department adhering to the service concept of"general practice before specialty",the clinic provides over 100,000 services per year with ample opportunity for clinical experience,teaching and resources for scientific research for the trainees.The gradual progression of"observe,practice under supervision,and practice independently"is realized in the three-year training program.They can gain hands on experience early in their training to care for patients from disease recognition to providing comprehensive care under the physical-psychosocial model.Trainees are scheduled to be in the hospital's outpatient clinic one half day a week regardless of their current rotation.This will allow them to develop relationships with other fellow residents,instructors and follow up on chronic patients.Senior residents will be responsible for mini-lectures and lead case discussions for junior residents under the supervision of their instructors.Outstanding residents will be sent to Hong Kong to broaden their view of health care services in another health care system and in particular,primary care service provision.
