ML联合CBS教学法在消化内科住院医师规范化培训教学中的应用

摘  要:目的探讨微课课堂(ML)联合病例导入式教学(CBS)在消化内科住院医师规范化培训教学中的应用效果。方法选取2020年12月—2022年8月轮转四川省人民医院消化内科的74名住院医师规范化培训学生(简称“住培生”),采用随机数字表法分为CBS组和ML联合CBS组,每组37名。CBS组住培生采用CBS教学法,ML联合CBS组住培生采用CBS联合ML教学。比较2组住培生的考试成绩、医患沟通技能评分、教学效果评分及教学满意度。结果2组住培生培训后消化内科疾病基础知识考试成绩比较,差异无统计学意义(P>0.05);ML联合CBS组住培生培训后消化内科疾病病例分析、临床技能考试成绩及总成绩均高于CBS组,差异均有统计学意义(P<0.05)。2组住培生培训前医患沟通技能评价量表(SEGUE)评分比较,差异无统计学意义(P>0.05);2组住培生培训后SEGUE评分均高于培训前,且ML联合CBS组住培生高于CBS组,差异均有统计学意义(P<0.05)。ML联合CBS组住培生各项教学效果评分均高于CBS组,差异均有统计学意义(P<0.05)。ML联合CBS组住培生教学满意度优于CBS组,差异有统计学意义(Z=2.966,P<0.001)。结论对消化内科住培生用ML联合CBS进行教学培训,能明显提升其对消化内科疾病基础知识的掌握程度、临床技能、病例分析能力及医患沟通能力,获得多数住培生的认可,有助于提高临床技能教学质量,值得在消化内科住院医师规范化培训中推广应用。Objective To explore the application of micro-lecture learning(ML)combined with case-based study(CBS)in the standardized training of residents in the.Methods A total of 74 standardized training residents(referred to as"residents")rotating in the Department of Gastroenterology of Sichuan Provincial People's Hospital from December 2020 to August 2022 were selected as participants,who were randomly assigned to the CBS group and the ML combined with CBS group,with 37 residents in each group.Residents in the CBS group were taught using the CBS teaching method;while the residents in the ML combined with CBS group,the CBS combined with ML teaching method.The examination scores,physician-patient communication skills scores,teaching effectiveness scores,and degree of satisfaction with teaching of the two groups were compared.Results No statistically significant difference was observed in the examination scores of basic knowledge of digestive diseases between the two groups after training(P>0.05).The scores of case analysis of digestive diseases,clinical skills examination,and total scores of the residents in the ML combined with CBS group were significantly higher than those in the CBS group(P<0.05).No statistically significant difference was found in the scores of Set the stage,Elicit information,Give information,Understand the patient's perspective,and End the encounter(SEGUE)scale between the two groups before training(P>0.05).The SEGUE scores of both groups were higher after training,and the residents in the ML combined with CBS group had significantly higher scores than those in the CBS group(P<0.05).The teaching effectiveness scores of the residents in the ML combined with CBS group were significantly higher than those in the CBS group(P<0.05).The degree of satisfaction with teaching of the residents in the ML combined with CBS group was significantly higher than that of the CBS group(Z=2.966,P<0.001).Conclusion Teaching and training among residents in the department of gastroenterology using ML combined with CBS ca
