翻转课堂结合OSCE模式在呼吸与危重症医学科教学中的应用

摘  要:目的探讨翻转课堂结合客观结构化临床考试(objective structured clinical examination,OSCE)模式在呼吸与危重症医学科临床教学中的应用价值。方法回顾性分析安徽医科大学第一附属医院呼吸与危重症医学科2019年1月—2021年6月83名实习医学生的档案资料,按照不同的教学方法进行分组,将2019年1—12月接受常规教学结合OSCE模式的医学生设为对照组(n=41),将2020年6月—2021年6月接受翻转课堂结合OSCE模式的医学生设为研究组(n=42)。比较2组的理论知识考核成绩、OSCE成绩、评判性思维能力和学习满意度。结果研究组的理论知识考核成绩(呼吸与危重症医学科疾病知识、诊断与病情评估知识、治疗知识)高于对照组,差异有统计学意义(P<0.001)。OSCE成绩(治疗问诊、治疗查体、疾病诊断、治疗方案、治疗实施、疗效评价、健康教育)高于对照组,差异有统计学意义(P<0.001)。教学后,研究组寻找真理、开放思维、分析水平、系统化水平、自信心、求知欲望、认知水平评分均高于对照组,差异有统计学意义(P<0.001);研究组的学习满意度高于对照组(97.56%vs.80.49%),差异无统计学意义(P>0.05)。结论翻转课堂结合OSCE模式的教学能够培养呼吸与危重症医学科实习医学生的评判性思维能力,提高其呼吸与危重症医学科的相关理论知识与技能操作水平,提高医学生的学习满意度。Objective To explore the application value of flipped classroom combined with objective structured clinical examination(OSCE)mode in clinical teaching of respiratory and critical care medicine.Methods A retrospective analysis was conducted on the files of 83 medical interns in the department of respiratory and critical care medicine of the First Affiliated Hospital of Anhui Medical University.Medical students who received routine teaching combined with OSCE mode from January 2019 to December 2019 were divided into a control group(n=41),and medical students who received flipped classroom teaching combined with OSCE mode from June 2020 to June 2021 were divided into a research group(n=42).Compare the theoretical knowledge assessment scores,OSCE scores,critical thinking abilities,and learning satisfaction between two groups.Results The theoretical knowledge assessment scores(respiratory and critical care medical disease knowledge,diagnosis and disease assessment knowledge,treatment knowledge)of the research group were higher than those of the control group,and the differences were statistically significant(P<0.001).The score of OSCE(treatment consultation,treatment physical examination,disease diagnosis,treatment plan,treatment implementation,efficacy evaluation,health education)were higher than that of the control group,and the differences were statistically significant(P<0.001).After teaching,the research group scored higher than the control group in terms of seeking truth,open thinking,analytical level,systematization level,self-confidence,thirst for knowledge,and cognitive level,and the differences were statistically significant(P<0.001).The learning satisfaction rate of the research group was higher than that of the control group(97.56%vs.80.49%),there was no statistical significance(P>0.05).Conclusion The combination of flipped classroom and OSCE mode can cultivate the critical thinking ability of respiratory and critical care medical interns,improve their theoretical knowledge and skill operation level relate
