摘 要:目的利用计算机辅助考试手段(CBT)与客观结构化临床考试(OSCE)测试全科医师综合实践能力,探索客观有效地测评全科医师实践能力、评价全科医师培训的教学质量的方法。方法对134名全科医师岗位培训学员和18名规范化培训学员进行CBT测试和标准化病人(SP)多站考试,并对测试结果进行比较。结果SP全科考试得分与CBT测试成绩呈正相关;CBT测试均分与SP考试总评分均分有相关性(r=0.475,P=0.047),与SP临床技能测试总评分均分亦有相关性(r=0.568,P=0.014)。结论CBT对全科医师实践技能的部分项目的测试是有效和可行的,可用于辅助测试全科医师实践能力;采用CBT方法测试全科医师实践能力的同时,应辅以SP评价其体检、问诊等操作性能力,评价结果将更为客观、全面。Objective To found an objective and effective method to assess the clinical practical competence for general practitioners (GPs) and teaching quality of general practice through application of computer- based testing (CBT) and objectively structured clinical examination (OSCE) to assess the GPs' practical skills. Methods Totally 134 in - service training GPs and 18 standardized training GPs were tested by CBT, and the results of tests were analyzed using Pearson correlation and multiple linear regression methods. Results The GPs OSCE test results and CBT test results were positively correlated ( F = 6. 440, P =0. 010). The average CBT score was positively correlated with the average score on SP ( r =0. 475, P =0. 047), and also with the average score of clinical skills on SP ( r = 0. 568, P = 0. 014). Conclusion CBT is effective and feasible for test of partial items for practical skills of GPs. It can be used as auxiliary for GPs' clinical practical competence. The CBT with GP as the auxiliary to assess the operative capacities in physical examination and inquiry will make the results more complete and objective.
